HOSPITALITZACI\u00d3 I INTERVENCI\u00d3 QUIR\u00daRGICA.<\/strong><\/div>\n<div style=\"text-align: justify\">Cal aportar, a m\u00e9s de la baixa, uninforme m\u00e8dic:<\/div>\n<div style=\"text-align: justify\">En cas d\u2019ingr\u00e9s superior a 24 hores, a l\u2019informe s\u2019especificar\u00e0 el dia i l\u2019hora<\/div>\n<div style=\"text-align: justify\">d\u2019entrada i sortida del centre.<\/div>\n<div style=\"text-align: justify\">Quan l\u2019ingr\u00e9s \u00e9s inferior a 24 hores, l\u2019informe d\u2019intervenci\u00f3 quir\u00fargica ha<\/div>\n<div style=\"text-align: justify\">d\u2019especificar clarament que \u00e9s necessari un per\u00edode de rep\u00f2s o rehabilitaci\u00f3<\/div>\n<div>domicili\u00e0ria igual o superior a 24 hores. (No cal explicar el tipus d\u2019intervenci\u00f3).<\/div>\n<div><\/div>\n<div><strong>2. MALALTIA GREU\/AGUDA.<\/strong><\/div>\n<div>A m\u00e9s de la baixa, a l\u2019exemplar de la baixa per lliurar a l\u2019empresa, cal aportar un<\/div>\n<div>informe m\u00e8dic que indiqui que es pateix una malaltia greu o b\u00e9 un proc\u00e9s agut que requereix rep\u00f2s domiciliari. (No cal especificar cap diagn\u00f2stic m\u00e8dic, per\u00f2 aquesta decisi\u00f3 es deixa a consideraci\u00f3 del treballador\/a.)<\/div>\n<div>Algunes d\u2019aquestes malalties greus o agudes estan incloses en el RD2210\/1995 (grip, bronquitis aguda, crisis asm\u00e0tica, hepatitis, rub\u00e8ola, varicel\u00b7la,etc.) i en el RD 1148\/2011 (conjunt de malalties greus o c\u00e0ncers).<\/div>\n<div id=\"pageContainer2\" class=\"page\">\n<div class=\"canvasWrapper\"><\/div>\n<div class=\"textLayer\">\n<div><strong>3. SUP\u00d2SITS ESPECIALS:<\/strong><\/div>\n<div>\u00danicament estan incloses les fractures i accidents no laborals que impossibilitin<\/div>\n<div>despla\u00e7ament al centre de treball o immobilitzaci\u00f3 prolongada.<\/div>\n<div>Cal portar la baixa i adjuntar-hi un informe m\u00e8dic que indiqui que l\u2019empleat pateix<\/div>\n<div>una fractura o accident no laboral que impossibilita aquest despla\u00e7ament o que comporta una immobilitzaci\u00f3 domicili\u00e0ria prolongada. \u00c9s imprescindible que les<\/div>\n<div>paraules \u201cfractures o accident\u201d apareguin a l\u2019informe.<\/div>\n<div><\/div>\n<div>Malalties infecto contagioses per via aer\u00f2bica d\u2019origen v\u00edric, bacteri\u00e0 o per fongs, com refredats, faringitis, angines, etc. \u00danicament tenen dret ala percepci\u00f3 del 100% de la IT els CINC PRIMERS DIES I DUES VEGADES L\u2019ANY com a m\u00e0xim, segons l\u2019apartat c del punt 3r. de la circular que regula aquestes millores, a excepci\u00f3 de determinats col\u00b7lectius laborals com el personal de cuina, assistencial, etc., que per la naturalesa de la seva feina no tenen cap restricci\u00f3.<\/div>\n<div><\/div>\n<div><strong>4. INDISPOSICIONS:<\/strong><\/div>\n<div>Les indisposicions d\u2019un dia no estan afectades per la regulaci\u00f3 de les baixes, \u00e9s a<\/div>\n<div>dir, no s\u2019han de descomptar del sou.<\/div>\n<div>Amb l\u2019acord del 27 de mar\u00e7 de 2015, la documentaci\u00f3 justificativa no ser\u00e0 necess\u00e0ria per a dos dies d\u2019indisposici\u00f3 en un any natural.<\/div>\n<div>(La impossibilitat d&#8217;assist\u00e8ncia haur\u00e0 d&#8217;estar notificada al responsable immediat o<\/div>\n<div>al referent de recursos humans).<\/div>\n<div><\/div>\n<div>Per la resta d\u2019indisposicions, podem:<\/div>\n<div>\u2022Aportar un justificant m\u00e8dic o de visita on ha de constar que cal fer rep\u00f2s 24 hores. El facultatiu ho pot posar a m\u00e0 si el centre d\u2019atenci\u00f3 no disposa de justificant d\u2019indisposici\u00f3 oficial.<\/div>\n<div>\u2022Aportar el comunicat de baixa\/alta d\u20191 dia. No \u00e9s necessari justificar el motiu de la baixa. (Acord de febrer de 2016).<\/div>\n<\/div>\n<\/div>\n<div id=\"pageContainer3\" class=\"page\">\n<div class=\"canvasWrapper\"><\/div>\n<div class=\"textLayer\">\n<div><strong>5. Q\u00dcESTIONS IMPORTANTS:<\/strong><\/div>\n<div>AMB LA BAIXA NO S\u2019HA D\u2019ENVIAR A RRHH CAP INFORMACI\u00d3 que no sigui la imprescindible sobre el problema de salutque l\u2019origina.<\/div>\n<div>Per tant, NO HEU DE REMETRE:<\/div>\n<div>Historials\/informes d\u2019alta per intervenci\u00f3, ingr\u00e9s hospitalari, d\u2019urg\u00e8ncies, etc., on<\/div>\n<div>consti tot el vostre historial i antecedents m\u00e8dics (esteu donant informaci\u00f3 molt confidencial). No heu de lliurar proves diagn\u00f2stiques, anal\u00edtiques, etc., per la mateixa ra\u00f3.<\/div>\n<div>AMB LA BAIXA QUE CORRESPON A L\u2019EMPRESA NOM\u00c9S CAL ENVIAR l\u2019informe m\u00e8dic, el justificant d\u2019ingr\u00e9s hospitalari si fos el cas, o l\u2019informe de cirurgia ambulat\u00f2ria on especifiqui clarament un dels sup\u00f2sits recollits a la circular que regula les millores de la prestaci\u00f3 econ\u00f2mica d\u2019incapacitat temporal del personal al servei de la Diba que us adjuntem i que cal tenir sempre a m\u00e0.<\/div>\n<div><\/div>\n<div>Aquest full informatiu \u00e9s nom\u00e9s un resum i NO RECULL TOTS EL SUP\u00d2SITS<\/div>\n<div>que es poden donar.
